Dextromethorphan abuse inducing psychotic reaction

Presents the case of a male (aged 35 yrs) residing in Saudi Arabia with confusion and psychotic behavior resulting from dextromethorphan (DM) abuse. While on a family visit to United Arab Emirates, the family described the S as nervous, irritable, and agitated, and talking to nonexistent persons. His behavior deteriorated; eventually, the S was brought to a psychiatric hospital under police restraint, and given treatment with antipsychotic drugs. The S had been self-administering DM daily for 3 yrs, but had consumed 3 times his normal dose prior to travel. The S had no history of psychotic illness or drug abuse, and responded well to treatment. 48 hrs subsequently, the S had improved dramatically, but showed no memory of events.
