MAOI withdrawal phenomena in the elderly.

Presents case reports of 3 elderly individuals (aged 72-75 yrs) with histories of depression who developed MAO inhibitor (MAOI) withdrawal symptoms resulting from drug therapy changes. MAOI withdrawal symptoms include severe anxiety, delirium, pressured speech, sleeplessness or drowsiness, hallucinations, and paranoid psychosis. Since most MAOI patients are elderly, psychiatrists may need to stop or reduce MAOI because of physical illness, medication needs, or surgery. The onset of dementia, increasing confusion, physical complications, and drug interactions may also necessitate MAOI cessation. Trials of alternative drug strategies not involving discontinued MAOI use should be considered. Inpatient admission should be considered when MAOI cessation is necessary, and the underlying depression and withdrawal symptoms require treatment.
